If you are a new or returning patient with a new problem, please schedule a New Patient Visit. We will need to cancel your appointment if you choose any other visit type as it does not leave enough time. Please read each practitioners specialties and schedule with the one that fits best.
Prices: Listed prices reflect our prompt pay discount when paying at the time of service. This is typically the maximum you would pay for the service if insurance deductibles are not met or the service or condition is not covered. Actual prices submitted vary and are typically higher.
Insurance: We are in network with BCBS PPO and Medicare only. For other coverage we will happily supply you with a superbill which you may submit to your insurance for direct repayment. Medicare covers a spinal chiropractic adjustment only which would reduce the listed prices with Dr Eric by $30 if this therapy is performed. Elizabeth is not in Medicare and unable to submit for services.
Late Cancelation: Appointments missed and not cancelled at least 24 hours in advance will still incur full charges.
COVID/Illness: If you are experiencing cold/flu symptoms please reschedule your appointment before the 24 hour cancelation policy is in effect to avoid late cancelation fees. If you have possibly been around anyone sick please wear or ask for a mask. We may still require masks following major holidays or travel. If you are not OK with these office policies please seek care elsewhere. Thank you.
